Remember to book your student an appointment with the family doctor this summer before the start of the new school year. All students are required to have completed medical forms by the beginning of the new school year.
To do this, go to your OpenApply account and:
- Print out the Physical Exam form for your family doctor to complete. Once completed by the doctor, this form can be handed in in August, or uploaded to OpenApply.
- Review your student’s Medical History form. If there are any new changes (new allergy, new medication), you can edit the form on OpenApply. However, please send an email to nurse@isl.edu.lv as well.
Please take this opportunity over the summer break to get these documents completed so that students are able to participate in all school activities. Paper copies of these forms can also be printed out from the list below. If you have any other questions, please contact nurse@isl.edu.lv.
